All residents in the Township of West Milford who have damage resulting from the weekend of the April 2 flooding must complete a Damage Assessment Form.  These forms will be available at the Clerks Office Monday through Friday, 8:30 am to 4:30 pm.  Once completed you may drop the form back at the Clerk's office or fax the form to the Office of Emergency Management at 973-728-2880.  To request a form by fax or mail contact the Office of Emergency Management at 973-728-2840.
